1. An application for income-related daily allowance, preferably for a period of four weeks, but at least two weeks.
  2. A certificate of your annual income from YEL, MYEL or TyEL insurance for the current and the three previous years, or for the duration of your insurance if you have had it for a shorter period. You can get the certificate from your pension company.
  3. Sales profit report -form. This is a statement of the financial situation of your business.
  4. Balance sheet for the last full financial year. If you have not been required to keep double-entry books, we need a list of assets (assets and liabilities).
  5. A revised tax card issued for benefits. Without a revised tax card, the minimum tax rate is 25%.
